We ask that on your initial visit to Tullos Family Dentistry, you allow additional time before your scheduled appointment to complete your new patient forms. However, to save time, you may wish to visit the Patient Forms section to download these forms and bring the completed forms the day of your appointment.

We at Tullos Family Dentistry have provided a convenient way for you to complete your new patient forms, prior to arriving for your appointment with Dr. Tullos. Simply print your forms, complete them and bring them in for your first appointment.
Established patients:
To meet regulations for compliance, our office requires up-to-date patient information. We ask that patients update all forms on a yearly basis. Established patients may print the forms, complete them, and bring them into the office. Please feel free to contact our office if you are unsure if you may be required to provide up-to-date information.
